When does an ICAP REQMOD request 'fail_open'?

Issue/Introduction
ProxySG supports 'fail_open' for ICAP REQMOD in these cases:
1. When an HTTP request has 8K or less data, not counting the http header size (includes all GET request).
2. Failed health check on the ICAP server.
3. Failure to establish a TCP connection to the ICAP server.
