'Save as' pop-up not displayed after ICAP scan complete

When attempting to download a file from a web browser, the 'save as' pop-up does not display even though the ICAP patience page indicates that the entire object has finished being downloaded and scanned.
The patience page progress indicator is a reflection of how much data has been sent to the ICAP server. If the ICAP server does not respond indicating the object can be served to the client, the end user will not get the pop-up.
For example, an ICAP server may exhibit significant delays in responding with a "204 - No Content" after the last data was sent to it. Once the "204 - No Content" is returned, the end user will then receive the pop-up.
